Insurance
As the patient, you are responsible for all charges for your dental care. Your insurance is a contract between you and the insurance company.

Our staff will be happy to complete and submit your dental insurance forms to help you receive the maximum payment from your insurance company. There is no charge for this service. Please bring your insurance forms with you at the time of your appointment, with the patient portion of the form completed.
